ONLINE CONSULTATIONS  - 

Online consultations enable you to contact a GP or other health care professional over the internet.  It can save you waiting for an appointment or coming to the surgery in person.  You a can tell us about your health using a smartphone, tablet or computer.  Click HERE to complete our online consultation request form. 

NHS APP -

You can use the NHS app to also book an appointment download the app on  https://www.nhsapp.service.nhs.uk/login select and access the NHS app to book an appointment at the surgery.

My GP APP -

You can download the app on www.mygp,io/app and book an appointment for the clinician of your choice.

Appointments can also be made by contacting reception on 01204 661600.  The times of our surgeries can be found under 'opening times'.

Please note that our Saturday morning slots are for pre-booked nurse appointments only and not for emergency issues.

Urgent Appointments

If you require a urgent on the day appointment then please contact reception on 01204 661600, you may not be able to see your usual Doctor but we will endeavour to ensure you see a clinician on the same day to access urgent consultation.

Appointments Evenings & Weekends

We can also offer a variety of appointments (GP, ANP, Practice Nurse, MSK) weekday evening 6.30 pm to 9.30 pm and Saturday's.  These appointments are held at Southbrook Surgery, Southbrook Grove,  Bolton, BL3 2DN.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.  You can cancel appointments using any of the appointment links listed above or alternatively contact the Reception team on 01204 661600.
