IMPORTANT UGENT PATIENT NOTICE REGARDING MEASLES 

NHS England urges those unprotected against measles to come forward for their vaccinations

Measles cases are continuing to rise across England, with a number of unvaccinated children and adults requiring hospital as infections take hold. 

This month, millions of parents and carers in England are being contacted by the NHS to book their children in for their missed measles, mumps, and rubella (MMR) vaccine as part of a major new campaign to protect children from becoming seriously unwell.

NHS figures show almost 3 million children under the age of 16 years are either unprotected or not fully protected and at risk of catching these serious and completely preventable diseases.

Measles is not just a childhood disease and can be serious at any age. If caught during pregnancy it can be very serious causing stillbirth, miscarriage and low birth weight and NHS bosses are also urging young adults to catch up on any missed doses before thinking about starting a family.

Two doses of the safe and effective MMR vaccine are needed for maximum life-long protection, with the first dose given around the child’s first birthday, and the second dose given at around three years and four months old.

However, anyone can catch up at any age on any missed doses and it’s never too late to protect yourself.

If you or your child have not had both dose of the MMR vaccine contact us today to book an appointment as soon as possible and catch up.

At Swan Lane Medical Centre in addition to our GPs we now have a multidisciplinary team of healthcare professionals you can now access including, 1st Contact Physiotherapist, Paramedic, Advanced Clinical Practitioner, Clinical Pharmacist and Social Prescribers.  

Mental Health Practitioner 

Mental health practitioners in primary care provide GPs and other primary care staff with timely support on the best ways to help people with mental health needs.

1st Contact Physiotherapist

Our 1st Contact Physiotherapists is a highly trained health professional who can help patients with musculoskeletal issues such as back, neck and joint pain by: assessing and diagnosing issuesgiving expert advice on how best to manage their conditionsreferring them onto specialist services if necessary.  To learn more please click here to watch a short video. 

Paramedic in General Practice 

Our Paramedic is a highly-trained health professional assesses and manages patients presenting with acute illness and those with long-term conditions in same-day service clinics. To learn more please click here to watch a short video. 

Advanced Clinical Practitioner

Our Advanced Clinical Practitioner is highly-trained health professionals who can undertake complex reviews of patients, similar to GPs. Social Prescriber Link Workers

Our team of social prescribers talk to patients about what matters to them, and what support they need to feel happier and healthier. GP Assistant

This is a dual role in which our GP Assistant can handle appointments and tests to free up the GPs time and also to assist with urgent tasks and administrative work.  To learn more about a GP Assistant click here.

Test Results 

Patients will be contacted by a member of the surgery team if their results are deemed abnormal, patients will then be advised if they need to book for a GP or Nurse to receive advice regarding their results or directed to the pharmacy if medication is required.  Patients can also contact the surgery and speak to a member of the reception team to be advised of their results, although we do ask that you don't contact the surgery before 11am to enable patients who need to see a GP can get through on the telephone lines.  You can also access using the NHS app on the link below:-

Medical Certificate - 'Sick/Fit' notes

Medical certificates or 'Sick/Fit notes need to be given to your employer if you have been ill for more than 7 days in a row and have taken 'sick leave'.  If you have been unwell and off for less than 7 days then you can 'self sign' your self sick and you will need to complete a self signing form that should be available from your employer.

You will need to be seen by or speak to a GP to request a 'sick note' if you have been off work for more than 7 days and your employer requires a Medical Certificate as your illness is ongoing.  This can be done by sending a request on line or by contacting the surgery on 01204 661600.  Alternatively if you need an ongoing 'sick note' please click here to complete an online request form, you may also do this via your NHS APP.  If you don't have the NHS app you can do this by visiting your app store on your mobile phone and downloading the free app.

PRIMARY CARE NETWORKS

Primary Care Networks (PCNs) are groups of practices that work together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas.

NHS England’s Long-Term Plan outlined the ambition for Integrated Care Systems. The BMA GP (England) committee and NHS England agreed, through national contract negotiations, to develop and roll out of Primary Care Networks (PCNs).

There are nine PCNs in Bolton.  Swan Lane Medical Centre is part of the Rumworth Primary Care Network.  Rumworth Primary Care Network is comprised of the following 4 practices

Swan Lane Medical Centre, Dr Mirza & Partners, Shanti Medical Centre and Garnet Fold practice.  As a Network we work collaboratively to improve patient access and extend the range of services available to you, and by helping to integrate primary care with wider health and community services.
