Clinics & Services

In addition to general medical consultations we are pleased to be able to provide specialist clinics and services covering the following areas:

• Chronic Disease Management - special clinics are run by the practice for the management of diabetes, coronary heart disease, heart failure, asthma, CKD, Hypertension and chronic pain. 
• Carers Health Checks - Our trained Health Care Assistant/Assistant Practitioner offer health checks to all of our patients who are carer's. 
• Child Development Clinic/Post Natal Review/Baby's 8 Week Immunisations - the surgery will send you and your baby an appointment for your baby's 8 week check up and your post natal review.  During this appointment we will also co-ordinate an appointment with our Practice Nurse for your baby's 1st immunisations. 
• Child Immunisation Clinics - held weekly/bi weekly/monthly with our experienced Practice Nurses. 
• Family planning - the practice provides a range of contraceptive services. Please discuss this with the doctors in a normal consultation. Coils and implants are available through designated Family Planning Clinics.
• Travel Health Clinic - Our Practice Nurse holds a regular travel health clinic for travel health advise and travel vaccinations.
• Minor Surgery - the surgery has a well equipped treatment room and the doctors are accredited to provide a range of minor surgical procedures. Appointments for a minor operation should only be made after an initial consultation with one of the doctors.
• Cervical Screening Service - All of our Practice Nurses are trained and taking cervical smear tests. 
• Flu Clinics - Each year during the months of September to March we hold dedicated flu vaccination clinics for all patients who eligible for a free flu vaccination. 
• NHS Health Checks - our Assistant Practitioner, Health Care Assistant and Health Improvement Practitioner offer NHS Health Checks for patients aged between 40 and 74 who do not have pre-existing conditions.
• Health Improvement Practitioner Service - At Swan Lane Medical Centre we have a dedicated Health Improvement Practitioner (HIP) whose aim is to support our patients who are at risk of developing diabetes or cardiovascular disease to live healthier lives by working with you to help you make small but important lifestyle changes.
• MSK Practitioner - Appointments are available at the surgery with our MSK Practitioner for patient who need help with bone and joint pain.
• Mental Health Practitioner - Appointments are available at the surgery with our Mental Health Practitioner who is experience in helping our patients find the right help who suffer with common mental health disorders such as anxiety, mood disorder, depression, particularly where these occur in patients with long term conditions. 
• Social Prescribing Link Worker - We can offer referral to our social prescriber.  Social prescribing works for a wide range of people including people with one or more long term conditions, those he need support with their mental health, lonely or isolated, complex social needs affecting their wellbeing. www.facebook.com/RumworthPCN
