PRIMARY CARE NETWORKS

Primary Care Networks (PCNs) are groups of practices that work together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas.

NHS England’s Long-Term Plan outlined the ambition for Integrated Care Systems. The BMA GP (England) committee and NHS England agreed, through national contract negotiations, to develop and roll out of Primary Care Networks (PCNs).

There are nine PCNs in Bolton.  Swan Lane Medical Centre is part of the Rumworth Primary Care Network.  Rumworth Primary Care Network is comprised of the following 4 practices

Swan Lane Medical Centre, Dr Counsell & Partners, Shanti Medical Centre and Garnet Fold practice.  As a Network we work collaboratively to improve patient access and extend the range of services available to you, and by helping to integrate primary care with wider health and community services.

Dear Patient, We are aware that currently everyone is going through a difficult time with many people being directly affected with Covid-19 either with self-isolation,  lock-down and Tier restrictions.  We would like to reassure you that we are providing telephone assessments both for acute illness and chronic conditions.  The hospital is continuing to provide suspected cancer assessments for people that have symptoms that may indicate underlying/sinister cause or malignancy.  Although this list is not all encompassing we would ask that you contact us as you normal would if you develop or are concerned about anything including:

- Unexplained weight loss,

- Concerns over altered bowel habit, especially if associated with blood,

- New lumps in the abdomen and abdominal pain,

- Coughs for more than 4 weeks,

- Coughing up blood,

- Swallowing problems especially if progressive,

- Blood in urine,

- Breast lumps,

- Post-menopausal bleeding,

- Any changes to moles that you are concerned about,

- Progressive headache especially if associated with other signs and symptoms,

- Jaundice (yellowing of skin),

- Unexplained bruising,

- Testicular lumps.
